Your reservation will be complete after we receive this form and a 1 hour ($70) deposit. You can make the deposit in person, by using a credit card over the phone (724-934-7529), or by sending a check to PYCO School of Music (150 Lake Drive, Suite 103, Wexford, PA 15090). Deposits are non-refundable but transferable up until 2 weeks prior to your event.
